HEALTH EQUITY INITIATIVES: We work collaboratively with diverse populations to develop & support programs and policies that promote health equity. The Department's Prevention Priorities Program has hired a dedicated staff person to strengthen the department’s capacity to promote health equity across multiple behavioral health areas such as tobacco, nutrition and physical activity. The strengthen families program uses a combination of Family-Based Nurses and paraprofessional to improve the outcome for children by a home visiting based model.

As our diverse populations expand, ensuring that all communities have the necessary "building blocks" for cultivating equitable opportunities that support healthy lifestyles becomes increasingly challenging. As numerous factors impact a community's ability to experience "health," the TPCHD and its partners are taking a multi-jurisdictional approach to promoting health equity throughout the county.
